Summary of Duties:

Following the care plan established by a Registered Nurse, provides clients with assistance in activities of daily living, up to and including personal care, home making tasks and helping in transportation needs (store, Dr. appointments, etc.). These services may be provided in the client's home, an assisted living facility, a skilled nursing facility or hospital. Every client is different and has different needs and we are looking to provide our clients with the BEST QUALITY of care.

You must be able to read, write and speak English. You must have a valid driver's license, reliable transportation and proof of vehicle insurance. You’ll have to pass a criminal background check and drug screen, too.

Prepare clinical and progress notes, including discharge paperwork. Inform the physician and other staff of your clients changing needs. Assign Home Health aides, as needed, to your patient. Oversee LPNs and Home Health aides providing services to your patient.

Skilled Home Health services are provided to our clients in their place of residence, whether it be in their home, in an assisted living facility and even in a hospital setting. Respectful caring for people in need of Home Health services is an essential part of who we are as caregivers.

DUTIES AND RESPONSIBILITIES:

Provide services in accordance with accepted standards of practice for physical therapy and agency standards.

Document all therapy interventions

Provide initial and ongoing assessment and evaluation of patient/family response to therapy

Develop and revise the POC in consultation with the physician and other care members

Provide therapeutic treatments and evaluation of equipment needs as applicable

Advise and consult with patients' family and other agency personnel

Complete documentation in a timely manner

Evaluate outcomes of care and accurately complete OASIS assessment

Provide discharge planning and evaluation

Supervise PTA's every 30 days and HHA every 14 days when appropriate

Participate in staff meetings and in-service programs as required

EDUCATION, TRAINING AND EXPERIENCE: Current state license and meets on of the following:

Graduated after successful completion of a physical therapist education program approved by one of the following:

The Commission on Accreditation in Physical Therapy Education (CAPTE)

Successor organizations of CAPTE

An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8 CFR 212.15 as it relates to physical therapists.

Passed an examination for physical therapists approved by the state in which physical therapy services are provided.

Minimum of 2 years experience preferred.

REQUIREMENTS/QUALIFICATIONS:

Must be able to read, write and speak English

Must possess good verbal and written communication

Must be able to meet deadlines and work under limited supervision

Must be able to climb stairs and stand/walk for at least 4 hours. Be able to reach, stoop, bend and kneel using proper body mechanics.

Ability to lift/push/pull 20-50 lbs. regularly.

Valid state drivers license and proof of motor vehicle insurance.

Availability to work evenings and weekends as necessary.

BLS certification
